Gum Disease Treatment in Raleigh, NC

Gum disease is the most under-diagnosed problem in dentistry, and busy Triangle schedules make it easy to let cleanings slide until something shows up. At our North Raleigh office we measure your gum health at every checkup and catch trouble while it is still simple to treat.

We Screen Your Gums at Every Visit

Most people with early or moderate gum disease have no idea they have it. The signs are quiet — a little bleeding when you brush, gums that feel slightly tender, breath that is off — and the damage builds slowly over years. By the time it is obvious (receding gums, loose teeth, bone loss on an X-ray), it is far harder to undo.

So at our Louisburg Road office we run a periodontal screening at every six-month visit: a slim probe measures the pocket depth between gum and tooth at six points around each tooth, and we note bleeding, recession, and tissue tone. Find it at the earliest stage and the fix is simple. Find it later and we still have a clear plan to stop it. Everything routine and moderate we handle in-house; the complex, surgical work goes to a periodontist we trust.

Gingivitis vs. Periodontitis

The whole game is which side of one line you are on. Gingivitis is the early, fully reversible stage: plaque and tartar inflame the gum tissue, so it bleeds and looks puffy, but the bone under the teeth is still intact. A proper cleaning plus better home care returns the gums to healthy pink, and no lasting harm is done.

Periodontitis is when that inflammation has moved past the gum into the supporting bone. The bone begins to shrink, gums pull back, and the pockets deepen and grow harder to clean. You cannot regrow that lost bone with ordinary treatment, so periodontitis is not reversible — but it is absolutely manageable, and the sooner we step in, the more bone and tooth we keep. The jump from gingivitis to early periodontitis often happens with no symptoms at all, which is exactly why the screening matters.

Making the First Visit Easy

A few small moves help a first appointment land well. Book a slot when your child is usually rested and fed — mid-morning beats a post-lunch or end-of-day appointment. Talk it up plainly beforehand (“tomorrow they’ll count your teeth and clean them”), and steer clear of loaded words like shot, hurt, or drill. Skip the promise that “it won’t hurt,” which only puts the idea on the table.

A kid-friendly dental video the night before helps younger children picture it. And you are welcome in the room — for as many visits and as many years as your child wants you there. Some ten-year-olds still like a parent in the chair; some three-year-olds are happy on their own. Both are fine.

Which Cleaning Do You Actually Need?

One reason gum care confuses people: “a cleaning” is really three different appointments, and which one you need depends on your pocket depths, whether you bleed on probing, and whether any bone has been lost. Here is the plain version.

Regular Cleaning

PROPHYLAXIS — HEALTHY GUMS
  • For healthy gums or mild gingivitis with no evidence of bone loss.
  • Removes plaque and tartar above the gumline before polishing the teeth.
  • Preventive treatment that is quick and typically requires no anesthetic.
  • Most patients have pocket depths between 1–3 mm with little or no bleeding.
Usually every six months.

Deep Cleaning

SCALING & ROOT PLANING — ACTIVE DISEASE
  • The recommended treatment for early to moderate periodontitis.
  • Removes plaque and tartar below the gumline while smoothing root surfaces.
  • Usually completed one section of the mouth at a time using local anesthetic.
  • Recommended when pockets reach 4 mm or deeper with bleeding or bone loss.
A one-time therapeutic treatment followed by maintenance.

Perio Maintenance

AFTER PERIODONTITIS — KEEPING IT STABLE
  • For patients with a history of periodontal disease after scaling and root planing.
  • Cleans above and below the gumline while measuring pocket depths at every visit.
  • Designed to prevent disease recurrence and maintain periodontal stability.
  • Different from a routine cleaning because ongoing periodontal maintenance is required.
Every three months, ongoing.

What We Handle In-House & When We Refer Out

The large majority of gum care is routine-to-moderate, and we do it right here in Raleigh:

  • Periodontal screening at every cleaning — six-point pocket readings plus bleeding, recession, and tissue checks.
  • Scaling and root planing — the deep cleaning above, by quadrant with local anesthesia.
  • Periodontal maintenance — the three-month cleanings that hold a stabilized case in place.
  • Antimicrobial therapy — targeted antibiotic placed in a pocket when it is clinically warranted.
  • Home-care coaching — the brushing angles, water flossers, interdental brushes, and rinses that fit your mouth.

We send the complex, surgical work to a periodontist: bone (osseous) surgery, soft-tissue grafting for heavy recession, regenerative procedures, laser-assisted treatment, and cases with several severely compromised teeth. We coordinate the referral, stay involved, and pick your maintenance back up once the specialist phase is finished — your history and monitoring stay in one place. A sudden gum abscess, meanwhile, is urgent; see our Raleigh emergency dental page.

Why Your Gums Matter Beyond Your Mouth

Periodontitis is not only a dental issue. A solid body of research ties moderate-to-severe gum disease to higher cardiovascular risk, worse blood-sugar control in people with diabetes, and complications in pregnancy. The likely link is chronic inflammation — bacteria from deep pockets entering the bloodstream and stoking inflammatory responses elsewhere. For a lot of our North Raleigh patients, treating the gums is one concrete step toward better overall health, not just a nicer dental checkup.

A Few Raleigh Cases (Anonymized)

Composite examples, not specific patients.

WAKEFIELD • CAUGHT EARLY

Early periodontitis found at a new-patient exam

A professional in their mid-forties had received routine cleanings for years without a complete periodontal screening. During a comprehensive evaluation, we discovered several 4–6 mm periodontal pockets with generalized bleeding. Scaling and root planing over two appointments, followed by three-month periodontal maintenance and improved home care, helped stabilize the condition and preserve healthy bone support.

NORTH RALEIGH • LAPSED MAINTENANCE

Maintenance that slipped during a move

A relocating executive missed regular periodontal maintenance visits for nearly a year while moving. The periodontal pockets began to deepen again, but with targeted retreatment, renewed maintenance appointments, and personalized home-care coaching, the disease was brought back under control.

WAKE FOREST • IGNORED BLEEDING

“My gums always bleed a little”

A patient had ignored bleeding gums for several years, assuming it was normal. A periodontal examination revealed moderate gum disease. In-office scaling and root planing, combined with improved brushing and interdental cleaning habits, eliminated the bleeding and stopped the disease from progressing without the need for surgery.

What Gum Treatment Costs in Raleigh

Scaling and root planing at our Raleigh office runs $200–$350 per quadrant; most people need two quadrants (one side) or all four (full mouth), treated across one or two visits with local anesthesia. Three-month periodontal maintenance runs $150–$250 a visit — close to a regular cleaning, with the added monitoring built in. Most dental plans cover periodontal therapy at roughly 70–80% after the deductible (versus 100% for a preventive cleaning), and many require documented disease before approving the periodontal rate — we supply that documentation as part of care. Without insurance, the O2 Advantage Plan discounts routine and periodontal visits, and CareCredit and Sunbit spread larger treatment into payments. We verify your specific coverage first.

Raleigh Periodontal FAQs

How do I know if I have gum disease?

The reliable way is a periodontal screening at a cleaning — pocket-depth readings around every tooth that flag disease before you feel anything. Early gingivitis often just shows up as bleeding when you brush or floss; more advanced periodontitis can bring recession, bone loss on X-rays, loose teeth, and lasting bad breath. Plenty of early cases have no symptoms at all.

A regular cleaning (prophylaxis) clears plaque and tartar above the gumline for healthy or mildly inflamed gums. A deep cleaning (scaling and root planing) also goes below the gumline and smooths the roots, and it is the standard treatment for early-to-moderate periodontitis — usually with local anesthesia and more time in the chair.

Gingivitis, before any bone loss, can be fully reversed with treatment and better home care. Periodontitis, once bone is lost, cannot be cured but can be stopped and held stable at its current level. The earlier we intervene, the more bone and tooth you keep.

If you have had moderate-to-advanced periodontitis, then usually yes — ongoing three-month maintenance keeps the condition stable, because the prior damage makes the disease more likely to return over longer gaps.

We handle scaling, root planing, and periodontal maintenance in-house. Surgical work — bone surgery, soft-tissue grafting, regenerative procedures — goes to a periodontist we work with regularly, and we resume your maintenance once that phase is done.

Yes. Research links moderate-to-severe periodontitis with higher cardiovascular risk, poorer blood-sugar control in diabetics, and pregnancy complications, most likely through chronic inflammation. Treating the gums removes one of those systemic inflammatory sources.
