Wisdom Teeth, and Where We Draw the Line

Third molars tend to arrive between 17 and 25, and when the jaw can’t fit them they stay buried or tilt into their neighbors — a setup for pain, swelling, and gum infections that flare at the worst times. The good news for most Raleigh teens and young adults: an erupted wisdom tooth, and plenty of impacted ones, come out comfortably right here.

But we don’t claim all of them. Some third molars sit so deep, or wind their roots so near the lower jaw nerve or the sinus above, that removing them safely is a job for an oral surgeon. We image every case in 3D specifically to catch that before treatment, and if your scan shows one of those higher-risk situations, we’ll be straight with you and refer rather than push our luck with your nerve. For the large majority, it’s a same-practice procedure.

You won’t feel the surgery — local anesthesia handles that — and for the anxiety that often comes before the appointment, we offer two simple tools: nitrous oxide, the laughing gas that relaxes you during the visit and clears within minutes afterward, and an oral anti-anxiety medication we can prescribe to take ahead of time. We don’t do IV sedation in our office, so the rare case that truly needs general anesthesia goes to a surgeon equipped for it.

Where we refer out. Being upfront about our limits protects you. Jaw-realignment surgery, facial-injury and fracture work, sinus lifts, and biopsies are a specialist’s job — we route them to an oral and maxillofacial surgeon, together with very deep or nerve-hugging wisdom teeth and any infection creeping toward the neck or turning firm. In those cases we arrange the referral and pass along your imaging so you’re not starting over.

Most dental insurance treats a needed removal — wisdom teeth included — as covered care, frequently paying a healthy share once your deductible is met. Because the fee shifts with how involved the tooth is, we skip a flat online figure, check your coverage, and lay the numbers out in writing before we begin. Can my teen get wisdom teeth out here?

In most situations, yes — the ones that have come through and plenty still buried, each planned from a 3D scan. The deepest teeth, or any riding the nerve or sinus, we route to an oral surgeon, and the scan shows that before we begin.

We numb the area fully, offer laughing gas through the visit, and can call in a calming tablet to take beforehand. Intravenous sedation isn’t something we do here.

Necessary removals and wisdom teeth are covered under most policies. After confirming your coverage we put the estimate in writing at the visit; CareCredit, Sunbit, and the O2 Advantage Plan stretch any remainder into payments.

We don’t — corrective jaw work, facial injuries, sinus lifts, and biopsies all go to a specialist. Our lane is wisdom teeth, removals, implants, and small grafts.

The first 48 hours are the worst of it, with most routines resumed inside a week. Leave the clot undisturbed that opening day and dry socket is unlikely.
